Indeed, financial institutions employee lots of lawyers to make sure the loan contracts will always work in their favor, no matter what. So if you are borrowing serious amount of money, better read that fine print...

Yes they do, but what would you expect them to do, they are just protecting themselves.

I think, you can, but then still you will have to pay something extra, because normally people don't do that.
